Make your check payable (no cash please) to Computer Club of HSV for $15.00 (includes all household members). The half-year dues are $8.00 starting July 1. Bring this application and your check to a meeting or mail to Ted Elzerman, 18 Sosegado Lane, HSV 71909.

If you are using Internet Explorer 8 browser, you may experience a printing problem whereas the check marks in the check boxes do not print.
